Requests for records (prescriptions, exam notes, receipts, etc.) should be submitted through your patient portal. In order that we may securely send your information to you, please complete the following steps to create your portal and send a request:

  • Visit
  • Click the blue “Sign Up Now” button
  • Complete the form and click “Create Account”
  • Once logged in, click “Secure Messaging” on the left-hand side of the screen
  • Click the “Compose Message” tab
  • Compose your message, then click “Send Message”
  • Please allow three business days for all requests to be processed. Requests made over the phone will be directed to the patient portal.If you are at another provider’s office and that provider is in need of your records, he/she may ask you to sign a record release, and then fax it to our office at (919)516-0080. We will process these requests as quickly as possible.
